A couple of my would-be guesses are long gone, but there's still one I don't think has ever seen the light of day - Bernard Herrmann's It's Alive turns 40 this year, and while I see It's Alive 2 on LP from time to time, I don't think I've ever seen the original. And Herrmann is awesome.
